Mr Chan Hong

Mr Chan Hong is currently an ambulatory care pharmacist who is involved in managing ambulatory clinics in areas of anticoagulation, heart failure and cardiovascular risk-factor control for post-myocardial infarction patients at the National Heart Centre Singapore (NHCS). He is also dual board-certified in pharmacotherapy and cardiology pharmacy.

With a keen interest in preventive cardiology, Mr Chan advocates for patient empowerment and education to improve disease prevention and public health. He is also currently involved in the formulation and review of patient medication-information leafl ets (PILs) for the SingHealth cluster and the national workgroup.
