Principal Clinical Pharmacist and Cardiology Specialist Pharmacist\nKhoo Teck Puat Hospital

Dr Grace Chang is a principal clinical pharmacist and cardiology specialist pharmacist from Khoo Teck Puat Hospital. She holds a Bachelor of Science (Pharmacy) and a Doctor of Pharmacy (PharmD) degree from NUS and is also a board-certified cardiology pharmacist. Dr Chang’s clinical responsibilities are in the care of cardiology patients across various settings. Her roles span from providing acute care in the coronary care unit to seeing patients in the ambulatory-based cardiac pharmacist clinic.  In both settings, she manages patients with acute and chronic cardiovascular conditions such as acute myocardial infarction and ischaemic heart disease, heart failure and dysrhythmias, as well as cardiovascular risk factors like diabetes and dyslipidemia. Aside from providing institution-based services, she also guest lectures with the NUS Department of Pharmacy in cardiology topics at the postgraduate level. She is also a preceptor for pre-registration trainees, and PharmD candidates on cardiology clerkship rotations.
