This track showcases how collaborative, cross-disciplinary efforts are transforming healthcare delivery across hospital, community, and industry settings. It highlights pharmacist-led initiatives such as Malaysia’s long-standing Diabetes Medication Therapy Adherence Clinic programme and Singapore General Hospital’s Advanced Prostate Cancer Clinic, where pharmacists play a key role in multidisciplinary care. In the Singapore community, the Health TAG (Test, Advise, Go!) initiative and pharmacist-administered flu vaccinations under the Ministry of Health’s sandbox demonstrate growing pharmacist involvement in preventive care. Hong Kong’s Primary Care Blueprint further illustrates how systems-level changes, such as community medication collection points can enhance care accessibility.
