Ms Saw Yashi
Ms Saw Yashi works as an inpatient pharmacist at the National University Hospital (NUH). She completed her cardiology postgraduate year two (PGY2) in the United States in 2016 and obtained her collaborative prescribing agreement (CPA) after completing the National Collaborative Prescribing Program from NUS in 2019. Her scope of practice includes general and critical cardiology, anticoagulation and heart failure. She runs the medication therapy management (MTM) cardiology clinic, where she titrates and optimises heart failure medications for patients with heart failure, and she also manages patients’ drug-related problems and polypharmacy. She is now heavily involved in heart failure and works closely with the heart-failure team in the hospital.
